Blackburn with Darwen Transition Plans - Preparation for Adulthood

The 2015 Code of Practice places emphasis on preparing young people with SEN and disabilities for adulthood in order to improve their life chances.

The areas of focus include: 

  • Employment (this can be paid or voluntary work) 
  • Independent living skills 
  • Friendships and being part of the community 
  • Being healthy

In order to ensure that the EHC process thoroughly prepares young people for adulthood we propose that the attached EHC Transition Plan is completed for every young person’s review from Year 9 onwards.

This will enable everyone working with the young person to remain focused upon the pathways that they are following into adulthood. 

Families and professionals need to be aware of 'transition planning' and 'preparation for adulthood' before a young person joins Year 9 at school, as some of these areas can be worked towards at a younger age.
