Lancashire and South Cumbria Keyworking Service
The aim of keyworking is to reduce the number of young people and adults from being admitted to specialist hospital/provision because of their mental health/distressed behaviour.
The Keyworking Service offers a short term designated keyworker to support a child, young person and their parents and carers. In addition, the Keyworking Service offers a comprehensive keyworking package of training.
Who do we work with?
Individuals aged 0-25 years who have a diagnosed of a learning disability and/or autism
An individual who is at risk of hospitalisation or is currently hospitalised or at risk of accommodation or care package breakdown
The individual must be on the Dynamic Support Database.
Individuals 18-25 Years: Referrals are completed via the Integrated Commissioning Board (ICB) and sent to the Keyworking Service, LSCft.
What will we do?
Help the family understand the individual’s needs and navigate their journey through the system
Bring practitioners together in supporting the individual and their family to develop skills and strategies that help them live life in a meaningful way
Identify, guide and refer to other services where needed such as carer support services, CAMHS
Facilitate effective communication between the individual, the family and the practitioners
Advocate and champion for the individual and their family in discussions around the provision of a person centred package of care as required
Deliver training to parent and carers and practitioners in relation to keyworking to increase confidence and resilience.
The Keyworking Team have a rich range of backgrounds and experience and as a Team are dedicated and determined in ensuring individuals have the right support at the right time.
