Adults Social Care: Learning Disability Team

Blackburn with Darwen's Learning Disability Team includes Social Workers and Reviewing Officers.

Our team supports young people (17+) and adults with learning disabilities, and their family/carers to identify and assess their care and support needs, so that we can  recommend appropriate support and packages of care.

In order to be eligible for this service, the young person or adult would need to have a diagnosed learning disability, and meet the eligibility criteria under the Care Act.

The team also work closely with Children with Disabilities Social Care Team and The Leaving Care Team, to provide transition support for 17 year olds with a Learning Disability, preparing for Adulthood.

Referrals regarding these young people with will come from the young people's services teams themselves

Referrals can also come from a range of individuals including other specialist social care teams within the local authority, the adult themselves, families who require support and from professionals within health or education.

As a part of the process, the allocated social worker will work closely with the individual and family, to identify needs:

  • care needs
  • daily living and social needs
  • life skills enablement
  • person centred planning
  • employment and training needs

Following this assessment the social worker will put a recommendation of support in place.

They may also make referrals to health colleagues, make referrals to multi-agency and universal services, and identify continuing healthcare eligibility.

There is a range of longer term services that can be offered which includes the following:

  • Direct payments
  • Commissioned services
  • Domiciliary Care
  • Social opportunities
  • Employment and Training opportunities,
  • Respite Breaks
  • Accommodation and support options.

The team also offers carers' assessments regarding respite support to informal carers.

Examples of support for carers:

  • respite vouchers to use at the local in-house respite facility – St Aidans
  • an offer of a direct payment to purchase further services and support
  • signposting to advice and support
  • referral to The Carer’s Service and Care Network Hub.

As a part of the work undertaken by the team it will be necessary for the designated social worker to work alongside various other teams within the council to support individuals with their given needs:
