Our Community & Neurodevelopmental Paediatrics team is commissioned by NHS Blackburn with Darwen Clinical Commissioning Group (CCG)to provide a range of services that may also be accessed by children and young people with SEND and their families.

What service does the team provide to SEND children and young people in BwD?

The service provides medical assessment and diagnostics within the expertise of Community Neurodevelopmental Paediatrics for the population of  East Lancashire and Blackburn with Darwen.

  • The service is available up to the age of 16.
  • Young people with complex disability who are attending special schools are seen up to age 19years.

This specialist area of Community and Neurodevelopmental Paediatrics covers two distinct broad areas of service:

  • Neurodevelopmental (Neurodisability and Neurobehavioural conditions)
  • Community (LAC and  adoption  medical assessments, aspects of Safeguarding  and medical advice for Special Educational Needs (SEND)

This service focuses on the multi-disciplinary diagnostic assessment for children with the following health needs:

Neurodevelopmental

  • Developmental problems diagnostic assessment – and medical management via CDC services (pre school) – see appendix 1
  • Developmental Co-ordination disorder diagnostic assessment (6-16 years)
  • Autistic Spectrum Disorders Diagnostic assessment  (5 -11 years)
  • ADHD diagnostic assessment in primary school aged children (5 -11 years)

Neurodisability

  • Cerebral Palsy diagnosis and  medical management  – 0-16 years.
  • Assessment of  ongoing effects of head injuries in children 0-16 years  (non acute)
  • Medical aetiological  investigation of school age children with moderate to severe learning disabilities.   (5-16 years)

Medical management of complex neurological or genetic conditions (in association with other specialist services).

Person Centred Care

For the majority of children accessing our service, care is provided in conjunction with other agencies and NHS trusts. Children should receive appropriate, high quality care that is integrated and coordinated around their health needs whilst taking into account any additional factors. We strive to try and achieve this where possible.

Notes

We hold Community Paediatric and Neurodevelopmental clinics at:

  • Royal Blackburn Hospital
  • Livsey All Age Children’s Centre
  • Shadsworth Childrens Centre
  • Barbara Castle Way Health Centre
  • Darwen Health Centre
  • We also provide clinic’s within some special schools, Integrated Neighbourhood Teams and Integrated Locality Teams

Referral Details

The service can be accessed by a process of referral.  The team currently receive referrals from a wide range of people including

  • GPs
  • Children’s social care
  • Paediatricians
  • CAMHS teams
  • School teachers
  • Sschool nurses

The service does not accept self-referrals.
 
Referrals other than from a GP need to be via an open and active CAF to ensure that early intervention and prevention has been addressed before referral.

  • Additionally, there should be evidence of Team around Child meetings to support the CAF on referral, and evidence of other support in place for families.
  • A brief outline of the health reasons for referral to Community Paediatric service should support the CAF on referral.

The teams can be contacted directly to gain advice, support or signposting.

The first point of contact for the parent/carer or child/young person to discuss something about their care would be to the practitioner involved in delivering their care.

If the young person or parent / carer has any urgent worries or urgent concerns, they are able to call the central teams on 01254 732852. 


Once referred, the appropriate team will consider whether the child or young person will benefit from the service.

The referrer will also be able to contact the service to discuss whether the child or young would benefit from service support.


Health Visitors, School Nurses, GPs, other CAMHS teams and other professional therapies will be involved in providing support for interventions for children and young people.
