Health and Wellbeing Passport
The Health and Wellbeing Passport can help the doctors and nurses who care for children and young people with extra needs know more about them, providing a better understanding of the support they require. It is designed to offer additional information such as likes and dislikes, for example favourite drink or how to calm them down.
The ‘About Me’ card is designed to support young people with additional needs to lead a more independent lifestyle. The card can be used to inform people they come into contact with on a daily basis of how they may need a little more help and assistance.
You can obtain your passport or ‘About Me’ card from the Children’s wards at both the Royal Blackburn Hospital and Burnley General Hospital. The passports also available by emailing: Jeanette Pearson – email@example.com
