Community Connectors
BwD Neighbourhood Teams include Community Connectors that manage and deal with community issues.
They are based at locality hubs across the borough. The Community Connectors are able to provide 1:1 support to families and individuals who may have various needs.
The service will help them to make the most of the community and local provisions via support i.e. appointments, gym.
The service is a short term service offering universal support.
It is accessed via Transforming Lives.
