Children's Disability Service - Apple Trees Hub (Social Care)
The Children Act 1989, Section 17 places a duty on local authorities to undertake an assessment of need for children identified to be a ‘Child in Need.’ The Act identifies a child with a disability to be a Child in Need.
The referral process is via the Referral and Information Co-ordinators and a Single Assessment of Need is carried out jointly by a Social Worker in the appropriate Area Team alongside a Child Support Officer with disability experience from The Hub. This is a co-ordinated approach to assessment.
If there are no presenting safeguarding concerns, and the main areas of need from assessment are around disability issues, the need for appropriate support and services, then the case is allocated to a Child Support Officer in The Apple Trees Hub, Children’s Disability Service. Examples of need may include short breaks, domiciliary support, outreach and independence for the young person.
On completion of the Assessment, the Child Support Officer will discuss which services would best meet your child’s needs and you are offered discussion around choices and services that best suit your family.
The Team work closely with the Adult Learning Disability Service to plan the transition from Children’s to Adult Services from 17 years onwards.
As long as your child is continuing to need and receive a service from the Local Authority Children’s Services, they will remain an open case under 17 Child in Need with an allocated worker to support your family.
If the Single Assessment identifies that your child does not have a need for a specialist service from the Local Authority, you will be supported to access other universal services in your local community.

Families can self-refer by contacting the Referral Coordinators on 01254 585585. Any referrals by a professional involved with the family must be via a CAF and sent to MASH, Duke St, Blackburn BB2 1HD

If your child has a disability they are entitled to a Single Assessment provided by Children's Social Care. The provision of a service or support is dependent upon the outcome of the assessment.
