Complex Care and Treatment Teams (CCTTs)
Complex Care and Treatment Teams (CCTTs) provide high quality support and treatment for people over the age of 16 years who are experiencing complex mental health and social care needs. With the support and care of the team, people will be assisted to maximise their quality of life by promoting independence, recovery and social inclusion. The person’s needs are assessed and the care plan is developed with the person and reviewed regularly. The person is allocated a named member of staff (a Care Coordinator) who will coordinate their care and will act as a key point of contact for the person and their family and carers.
Complex Care and Treatment Teams consist of a variety of staff groups including;

•Occupational Therapists
•Support Workers
•Pharmacists
•Specialist Practitioners

Referrals to CCTTs are made from other teams within Lancashire Care NHS Foundation Trust, such as Single Point of Access, Crisis Resolution and Home Treatment Teams, In-Patient Units and Early Intervention Services.
CCTTs do not accept self-referrals. However referrals can be made by a person’s GP, but they will be assessed initially by the Single Point of Access Team to ensure they are referred to the correct team.
There is no charge for users of CCTTs.
